Admission Requirements
To ensure a safe and structured recovery environment, participants are required to meet the following criteria and provide necessary documentation during the admissions process.
1. Government IDentification
Valid driver's license or state-issued photo ID.
2. Social Security Number
Required for program enrollment and coordination with services when applicable.
3. Financial Documentation
Verification of income, benefits, or documentation financial support.
4. Medical Records
Recent treatment records or a physician's statement when applicable.
5. Medication information
A current list of prescription medications and known allergies.
6. Health screening documention
Recent TB test results or a negative chest X-ray, as required.
7. Emergency Contact Information
Valid contact information for a designated emergency contact.
